Sound Bath Attendance Sheet

As explained to you during your professional training here at Irish Academy of Sound Therapy, as Practitioners of Therapeutic sound working with groups or individuals, indoors or outdoors, there are certain legal obligations we must uphold in order to protect the public we are working with, to protect ourselves as practitioners, and to protect the Therapeutic Sound profession as a whole.

I have been made aware of a small number of sound bath facilitators in Ireland requesting their sound bath participants to complete a medical questionnaire before the sound bath begins. This belief demonstrates a fear which has no place in sound & vibrational therapies including sound bath.

If there will be no sound therapy instruments placed on the body, a Sound bath is regarded as a meditation for deep rest and relaxation. Any qualified Yoga teacher or other person qualified to teach Yoga Nidra or any other meditation can legally create a sound bath without any additional training, provided they are adequately insured as a Yoga Teacher, because sound bath is known is Vedic traditions as Nada Yoga, both the struck and unstruck sound.

What’s required to satisfy legal obligations and insurance requirements, is an attendance sheet which every participant should write their name and phone number. The attendance sheet should include a description of what will happen during a sound bath, along with a declaration of consent to participate in the sound bath.

To assist you with your legal obligations, I have provided my own Attendance Sheet as an example for your professional use under the terms of the Creative Commons with Attribution License. This means as a Yoga Teacher or other Holistic Health Professional you are Licensed free of charge to copy and re-use these forms as you wish for the following purpose:

Attendance sheet is essential for legal reasons as follows – should an individual pursue legal action at any time in the future (here in Ireland they have two years to take a case) The attendance sheet is your professional record that the individual attended (or didn’t attend) your sound bath on the alleged date and time.

Before publishing my attendance sheet here for download, I sent it to my insurance company for verification. No changes were necessary, as they could not find any fault with my attendance sheet so you can be fully confident that this attendance sheet meets our statutory requirements current at the time of publication. Please feel free to verify my attendance sheet with. your own insurance company and let me know what they advise.

Zoom Q&A Refresher

Zoom Refresher Q&A sessions are held regularly free of charge, for all students past and present and all who are interested regardless of where trained. Held once a month generally on the 2nd or 3rd Thursday of the month.
